Radiolabelled ALS Candidate Tests Brain Penetration in PET Trial

NRG Therapeutics has begun a PET imaging biodistribution study of its lead candidate NRG5051, using a radiolabelled version of the molecule in healthy volunteers to quantify how much of the drug reaches brain tissue. The compound inhibits the mitochondrial permeability transition pore through NLRX1, with the aim of preserving mitochondrial function in neurodegenerative disease. A Phase 2 proof-of-concept trial in amyotrophic lateral sclerosis is planned for 2027.

Key Points

  • PET scans measure NRG5051 penetration into brain tissue in volunteers
  • Drug targets mitochondrial permeability transition pore via NLRX1 inhibition
  • Phase 2 trial in ALS patients planned for 2027

Longevity Analysis

Mitochondrial permeability transition pore opening sits upstream of the energy failure and cell death seen across neurodegenerative conditions, making it a target that addresses a driver rather than a downstream symptom. Measuring brain exposure directly with radiolabelled compound before efficacy testing replaces assumption with quantified evidence about whether a molecule reaches the tissue it is meant to protect. The same mitochondrial decline underlies much of age-related loss in neural, cardiac and muscular tissue, which positions pore-stabilising strategies well beyond a single rare disease indication.
