Three-year follow-up from the Phase II ALTITUDE study showed that a single in-office suprachoroidal dose of Regenxbio's Surabgene (lomparvovec) produced durable regression of non-proliferative diabetic retinopathy, with 6 of 10 evaluable participants at the highest dose achieving greater than two-step improvement on the Diabetic Retinopathy Severity Scale without additional treatment. No vision-threatening events occurred, and no intraocular inflammation was observed through three years across 17 participants receiving short-course prophylactic topical steroids. The dose is now advancing into the Phase IIb/III NAAVIGATE trial.
Key Points
- Six of ten patients gained two-plus DRSS steps at three years
- Single suprachoroidal dose; no additional retinopathy treatment required
- No intraocular inflammation or vision-threatening events through 36 months
Longevity Analysis
Diabetic retinopathy is microvascular disease made visible, and the retina remains the only site where capillary deterioration can be graded directly and non-invasively. A one-time intervention that holds structural regression for three years shifts chronic microvascular management from repeated injections toward sustained endogenous protein expression, reducing the treatment burden that so often erodes adherence in long-horizon disease. The absence of intraocular inflammation with only short-course topical prophylaxis is equally consequential, indicating that delivery route can limit the immune reactivity that has constrained ocular gene transfer.
