Gene Therapy Targets Injection Burden in Diabetic Macular Edema

4DMT has begun enrolling in 4SIGHT, a 514-patient Phase 3 trial testing the intravitreal gene therapy 4D-150 against aflibercept dosed every eight weeks in treatment-naïve diabetic macular edema, with non-inferiority in visual acuity at Week 52 as the primary endpoint. Two-year data from 22 patients in SPECTRA Part 1 showed a mean gain of 10.8 letters, a 176 µm reduction in central subfield thickness, no intraocular inflammation or ocular serious adverse events, and marked reductions in supplemental injections. The therapy carries FDA Regenerative Medicine Advanced Therapy designation.

Key Points

  • Phase 3 trial enrolls 514 treatment-naïve diabetic macular edema patients
  • Two-year data: +10.8 letters, −176 µm retinal thickness, no inflammation
  • Single administration substantially reduced supplemental anti-VEGF injection frequency

Longevity Analysis

Diabetic macular edema is a downstream consequence of sustained metabolic dysregulation on the smallest vessels, and the retina remains one of the few places where microvascular deterioration can be observed directly and repeatedly. A one-time genetic instruction that prompts ocular tissue to manufacture its own anti-VEGF protein shifts the treatment model from repeated intervention to sustained endogenous production — relevant far beyond ophthalmology, since adherence to monthly injection schedules degrades over years and vision loss often follows the lapse rather than the disease. Secondary endpoints tracking two-step improvement in retinopathy severity will indicate whether the approach halts progression or meaningfully reverses established microvascular damage.
