Frailty Predicts Stroke Outcome But Not Treatment Benefit

A post-hoc analysis of the INTERACT3 trial found that frailty predicted worse outcomes after acute intracerebral haemorrhage in a graded fashion, with associations persisting after adjustment for age, neurological severity, and other prognostic factors. Frailty did not diminish the benefit of the protocolised care bundle; mortality reductions favoured the bundle across all frailty strata, with the largest absolute reductions among the frailest patients. Frailty therefore functions as a prognostic marker rather than a reason to withhold intensive acute treatment.

Key Points

  • Frailty graded predictor of poor outcomes after intracerebral haemorrhage
  • Care bundle reduced mortality across all frailty strata
  • Largest absolute mortality benefit occurred in frailest patients

Longevity Analysis

Frailty reflects accumulated loss of physiological reserve across circulation, muscle and bone integrity, immune competence, and neurological resilience — and it forecasts how the body responds to acute vascular injury. The clinical implication is that reserve should be measured and tracked as a modifiable trajectory long before an emergency, rather than used retrospectively to temper treatment intensity. Protocolised, consistently executed care delivered rapid benefit precisely where reserve was lowest, underscoring that disciplined implementation of a standard protocol often outperforms individualised deviation from it.
