Resolution Therapeutics has completed dosing in EMERALD, a first-in-human Phase I/II trial of RTX001, an engineered macrophage therapy for end-stage liver disease with prior decompensated cirrhosis. The cells are modified to express IL-10 and MMP-9, targeting the inflammatory and fibrotic processes that disable the liver's own repair signalling. Fifteen patients across UK and Spanish hepatology centers were treated by standard peripheral infusion, with interim data expected in the fourth quarter of 2026.
Key Points
- Final patient dosed in first-in-human engineered macrophage cirrhosis trial
- IL-10 and MMP-9 target inflammation and fibrosis simultaneously
- Outpatient IV infusion under 10 minutes, no lymphodepletion
Longevity Analysis
Most interventions in age-related organ decline aim to slow deterioration; this approach attempts to reinstate a repair program the tissue has already lost. Macrophages sit at the intersection of immune surveillance and tissue remodeling, and restoring their reparative phenotype in a fibrotic organ tests whether late-stage structural damage remains reversible rather than merely manageable. The delivery profile — outpatient infusion, no conditioning regimen, a 40% reduction in cost of goods — determines whether regenerative cell therapy becomes a scalable clinical option or remains confined to specialist centers.
