Resolution Therapeutics has finished dosing 15 patients in EMERALD, a Phase I/II trial of RTX001, an engineered macrophage therapy expressing IL-10 and MMP-9 for end-stage liver disease following hepatic decompensation. The cells are delivered by a short peripheral infusion without lymphodepletion, pre-conditioning, or bridging therapy. Interim data are expected in the fourth quarter of 2026, with a pivotal U.S. study planned for 2027.
Key Points
- Fifteen end-stage liver disease patients dosed with engineered macrophage therapy RTX001
- Cells engineered to express IL-10 and MMP-9 targeting fibrosis
- Outpatient infusion under 10 minutes, no lymphodepletion required
Longevity Analysis
Fibrosis is the common endpoint of chronic inflammatory injury across most organs, and the liver offers the clearest testing ground for whether cell-based intervention can reverse rather than merely slow scarring. Engineering immune cells to carry both an anti-inflammatory signal and a matrix-remodeling enzyme targets the interference — persistent inflammatory drive and accumulated fibrotic tissue — before attempting to restore functional capacity, an approach with implications for any organ where repair has been overwritten by scar. Delivery without lymphodepletion or conditioning matters practically: it lowers the physiological cost of treatment in a population with minimal reserve, and makes repeat dosing conceivable.
