Cholinesterase inhibitors remain the long-standing pharmacologic standard in Alzheimer's disease, yet their measured effect on cognition is modest and does not alter the underlying course of neurodegeneration. The distinction between temporarily supporting neurotransmission and preserving neuronal tissue defines both the ceiling of these drugs and the case for addressing modifiable risk decades earlier.
ZNF865 (BLST), a previously uncharacterized zinc finger protein, has been identified as a regulator of genomic stability and cellular senescence in the intervertebral disc. Its expression declines with age and degeneration in human and mouse discs, and experimental suppression drives senescence, DNA damage, disc degeneration, and pain behavior in rodents, while restoration reverses senescent features and restores chromatin accessibility toward a healthy state. The finding links a discrete molecular control point to both tissue aging and the clinical experience of pain.
Genentech has acquired exclusive worldwide rights to Alector's AL050, an engineered glucocerebrosidase (GCase) enzyme replacement therapy for Parkinson's disease, for $100 million upfront and up to $1.17 billion in milestones. The candidate targets deficient lysosomal lipid clearance in neurons, a mechanism strongly associated with GBA1 mutations, and is paired with a proprietary carrier designed to cross the blood-brain barrier. AL050 has not yet entered clinical trials, and no human or delivery data were disclosed.
Swedish biotech Alzinova will present the design of its global Phase IIa ADVANCE trial for ALZ-101, a therapeutic vaccine targeting toxic amyloid-beta oligomers, at the Clinical Trials on Alzheimer's Disease conference in November. The randomized, double-blind, placebo-controlled study carries a primary endpoint of therapeutic effect rather than safety alone, with first enrollment expected around the turn of 2026 into 2027. A vaccine approach to amyloid, if validated, would represent a more durable and less resource-intensive alternative to the antibody infusions currently dominating the field.
Cognition Therapeutics will meet the FDA on 20 October to finalize how success is measured in a planned Phase 3 trial of zervimesine (CT1812) for dementia with Lewy bodies, with psychosis — hallucinations and delusions — proposed as the primary endpoint. The agency has previously indicated psychosis could serve both as an approvable outcome and as a marker of disease progression. In the earlier Phase 2 SHIMMER study, the company reported an 89% slowing of progression in hallucinations and delusions versus placebo on the neuropsychiatric inventory.
In SAMP8 mice, a model of accelerated aging, males and females followed distinct trajectories of hippocampal neurogenesis despite comparable cognitive performance at six months. The diterpenoid compound ER272 was associated with improved cognition and preservation of neurogenic parameters in males only, with limited effect in females. The findings indicate that sex modifies both the biology of brain aging and the response to interventions aimed at slowing it.
Jupiter Neurosciences has dosed the first patient in RESET, a Phase 2a randomized, double-blind, placebo-controlled trial of JOTROL — a micellar trans-resveratrol formulation — in approximately 30 Parkinson's disease patients over 12 weeks. Primary endpoints are safety, tolerability and pharmacokinetics, with exploratory biomarkers covering energy metabolism and inflammation. Phase 1 data reportedly showed increased systemic exposure and measurable cerebrospinal fluid concentrations without serious adverse events.
In the Phase 3 ARISE trial, once-daily oral solengepras 150 mg reduced average daily OFF time by 0.61 hours versus placebo at week 12 and increased ON time without troublesome dyskinesia by 0.60 hours in people with Parkinson's disease and motor fluctuations. Secondary measures including daily function (MDS-UPDRS Part II), daytime sleepiness and quality of life also favored the active arm, with 91% trial completion and a 3.5% discontinuation rate due to adverse events. Cerevance plans to discuss a New Drug Application with the FDA.
A network analysis of rural left-behind older adults in China examines how perceived care is structured after adult children migrate for urban work. Rather than treating care as a single measure, the method maps which components of felt support hold the most central position within the broader experience of being cared for. Perceived support quality is a consistent correlate of late-life morbidity and mortality, making its internal structure relevant to how aging trajectories diverge within the same material conditions.
Across two large longitudinal cohorts totaling more than 468,000 adults in Israel and a matched US network, serum 25-hydroxyvitamin D below 10 ng/mL was associated with substantially higher all-cause mortality (adjusted HR 2.57 and 1.59) and elevated risk across metabolic, cardiovascular, renal, neurodegenerative, and microvascular outcomes. Pharmacy-dispensed supplementation modeled longitudinally showed dose-dependent risk reduction, with a mortality hazard ratio of 0.72 for exposure equivalent to raising serum levels by more than 10 ng/mL. Marginal structural Cox models with inverse-probability weighting produced results consistent with conventional adjustment, strengthening the case for a modifiable contribution.
A brief four-session group intervention combining socio-medical education with emotion-focused coping strategies was evaluated in caregivers of people with dementia receiving psychiatric care. The proof-of-concept design points to reduced psychological strain in a population carrying a well-documented excess burden of stress-related morbidity. Caregiver distress is a modifiable exposure with measurable downstream consequences for health span.
Life Biosciences has reported the first human data from a Phase 1 trial of ER-100, an AAV2-delivered gene therapy expressing three Yamanaka factors (OCT4, SOX2, KLF4) in the eye under doxycycline control. Across three patients with open-angle glaucoma at the starting dose, no serious adverse events or dose-limiting toxicities occurred through Day 56, and two participants showed early improvement on visual-field testing. Partial epigenetic reprogramming has moved from animal models into a controlled clinical question, with dose escalation now authorized.
